摘要:
目的 了解主动脉夹层术前家属代理决策现状并分析其影响因素,为后续构建决策辅助方案提供参考。方法 2021年2-7月,便利抽样法选取湖北省武汉市某三级甲等医院心脏大血管外科的参与主动脉夹层术前代理决策的家属110名为研究对象,采用一般资料调查表、决策参与量表、决策冲突量表对其进行问卷调查。 结果 主动脉夹层患者家属决策冲突总分(40.44±8.90)分,98.19%的研究对象存在决策冲突; 多元逐步回归分析显示,家庭月收入、与患者感情状况、文化程度是主动脉夹层术前家属决策困境的影响因素(均P<0.05)。结论 主动脉夹层患者家属决策冲突明显且受多种因素影响,医护人员需在重视其内心体验的基础上,借助个性化的决策辅助方案降低决策冲突,提高决策质量。
Abstract:
Objective To explore the status quo of family agency decision-making before aortic dissection(AD)operations and its influencing factorsl, and to provide reference for the future decision-making plan.Methods From February to July 2021,a total of 110 family members who participated in the preoperative interview of AD operation in the Department of Cardiovascular Surgery in a tertiary A hospital in Wuhan,Hubei province were selected by convenience sampling method and investigated by the family general information questionnaire,CPS and DCS.Results The total score of decisional conflict among family members of patients with aortic coarctation was(40.44±8.90),and 98.19% of the study subjects had decisional conflict.Multiple stepwise regression analysis showed that monthly family income,relationship status with the patient,and education level were the influencing factors of preoperative decisional dilemma of family members with aortic coarctation(all P<0.05).Conclusions Decision-making conflicts among family members of patients with aortic coarctation are obvious and influenced by a variety of factors.Healthcare professionals need to reduce decision conflicts and improve the quality of decision-making with the help of personalized decision support programs based on their inner experience.
