摘要:
目的 探讨静脉血栓栓塞症(venous thromboembolism,VTE)患者出院后抗凝治疗依从性现状及其影响因素。方法 2020年9月至2021年2月,采用便利抽样法选取上海某三级甲等医院血管外科确诊为VTE的出院患者190例作为研究对象,使用一般情况调查表、Morisky用药依从性量表8条目(Morisky medication adherence scale,MMAS-8)进行问卷调查。结果 VTE患者出院后抗凝治疗依从性总分为(7.56±0.56)分; 依从性好患者56例(29.5%),依从性一般患者134例(70.5%)。多元回归分析结果显示,影响VTE患者出院后抗凝治疗依从性的因素为是否退休、文化程度、是否知晓抗凝药物名称、抗凝药物每日用药频次、抗凝药物每日使用剂量、用药期间未关注是否出现出血(均P<0.05)。结论 VTE患者出院后抗凝治疗依从性有待进一步提高,医护人员应结合相关影响因素强化出院后抗凝治疗管理,以提高患者依从性。
Abstract:
Objective To explore the anticoagulation therapy adherence and analyze its influencing factors among patients with venous thromboembolism(VTE)in post-discharge.Methods By convenience sampling method, 190 patients with VTE in post-discharge from one tertiary hospital in Shanghai were selected from September 2020 to February 2021.The general information questionnaire,Morisky Medication Adherence Scale(MMAS-8)were used for investigation.Results The total score of anticoagulation therapy adherence of VTE patients after discharge were 7.56±0.56.About 29.5% of patients(56 cases)had good adherence,70.5% of patients(134 cases)had mediocre adherence.Regression analysis showed that the influencing factors of the anticoagulation therapy adherence among patients with venous thromboembolism in post-discharge were whether they retired,education level,whether they knew the name of anticoagulant,daily administration,daily dose and whether they paid attention to bleeding during medication(all P<0.05).Conclusions The anticoagulation therapy adherence among patients with VTE in post-discharge needs to be improved urgently.Medical staff should strengthen the management of anticoagulant treatment after discharge in combination with relevant influencing factors to improve patients' compliance.
