摘要:
目的 了解疾病接受度和家庭关怀度对老年共病患者心理一致感的影响,以期为改善老年患者心理健康状况提供依据。方法 2020年9月至2021年2月,采用便利抽样法选取某三级甲等医院老年病科住院治疗的老年共病患者444例为研究对象,采用一般情况调查表、心理一致感量表、疾病接受度量表、家庭关怀度量表对其进行调查。结果 老年共病患者心理一致感总分为(57.13±14.02)分,处于低水平。疾病接受度、家庭关怀度越高的老年共病患者,其心理一致感总分越高(均P<0.05)。疾病接受度、家庭关怀度与老年共病患者心理一致感总分呈正相关(r=0.648、0.562,均P<0.05)。分层回归分析显示,年龄、文化程度、月收入、患病数量、疾病接受度、家庭关怀度是老年共病患者心理一致感的影响因素(均P<0.05),其中疾病接受度、家庭关怀度能解释老年共病患者心理一致感总变异的14.1%(ΔR2=0.141,P<0.001)。结论 老年共病患者心理一致感水平偏低,医护人员可通过提高疾病接受度和家庭关怀度水平,进一步提高老年共病患者心理一致感水平。
Abstract:
Objective To investigate the effect of illness acceptance and family care on the sense of coherence of elderly patients with comorbidities,and to provide basis for promoting the mental health of the elderly.Methods From September 2020 to February 2021, 444 elderly patients with comorbidities were selected from Department of Geriatrics of a tertiary A hospital by convenience sampling method.And a survey was conducted by using the general information questionnaire, Sense of Coherence Scale, Illness Acceptance Scale, and the family care scale.Results Total scores of sense of coherence in elderly patients with comorbidities was(57.13±14.02), which was at a low level.Illness acceptance and family care were positively correlated with the total scores of the sense of coherence of elderly patients with comorbidities(r=0.648,0.562,all P<0.05).Hierarchical regression analysis showed that age, education level,monthly income,number of illnesses,illness acceptance,and family care were the influencing factors of mental coherence in elderly patients with comorbidities(all P<0.05),among which illness acceptance and family care explained 14.1% of the total variation in the mental consistency of elderly patients with comorbidities(ΔR2=0.141,P<0.001).Conclusions The sense of coherence in elderly patients with comorbidities is at a low level.Medical staff can further improve the level of sense of coherence in elderly patients with comorbidities by improving the illness acceptance and family care.
