摘要:
目的 探析老年脑卒中患者居家健康管理的影响因素。方法 2019年12月至2020年1月,采用目的抽样法选取上海市静安区某综合医院和社区卫生服务中心的13名医务人员、9名老年脑卒中患者及7名照顾者为研究对象,对其进行半结构式访谈,运用定向内容分析法对资料整理、分析、提炼、归纳主题。结果 老年脑卒中患者居家健康管理影响因素可归纳为患者因素、照顾者因素、医疗卫生服务因素及政策因素4个类属、10个主题。结论 实施居家健康管理需加大力度落实分级诊疗,积极提供政策支持,合理优化人力资源配置,设置专职岗位,同时不断提高服务质量,完善相关基层医疗设施,提升患者自我管理及健康自信,充分发挥照顾者角色担当。
Abstract:
Objective To explore the influencing factors of home health management of elders with stroke.Methods From December 2019 to January 2020,13 medical staff,9 elders with stroke and 7 caregivers from a general hospital and community health service center in Jing'an District of Shanghai were selected as the research subjects by the objective sampling method.Semi-structured interviews were conducted,and the data were sorted,analyzed,refined and summarized by directional content analysis method.Results As for the home health management of elders with stroke,the influencing factors can be summarized into four categories,namely,patient factor,caregiver factor,medical and health service factor and policy factor,with nine themes.Conclusions The implementation of home health management needs to strengthen the implementation of hierarchical diagnosis and treatment,actively provide policy support,reasonably optimize the allocation of human resources,set up full-time posts,continuously improve the service quality,improve the relevant primary medical facilities,enhance the self-management and health self-confidence of patients,and give full play to the role of caregivers.
