摘要:
目的 探讨失效模式与效应分析(failure mode and effect analysis,FMEA)在预防减重手术患者压力性损伤中的应用及效果。方法 便利抽样法选择2018年1月至2019年12月在某院行减重手术治疗的病态性肥胖症患者及其手术护士为研究对象。将2018年行减重手术者(共34例)作为对照组,采用常规压力性损伤护理流程; 将2019年行减重手术者(共41例)作为观察组,采用FMEA优化后的压力性损伤护理流程,对筛选到的高危失效模式进行有针对性的整改。统计分析两组高危失效模式风险优先指数(risk priority number,RPN)、高危失效模式的发生率和患者压力性损伤发生率的变化。结果 在FMEA优化后,观察组高危失效模式的RPN值降低,高危失效模式的发生率和患者压力性损伤发生率均显著降低(均P<0.05)。结论 FMEA能够有效筛查出影响减重术压力性损伤发生的高危失效模式,并对护理流程进行优化,从而提高护理质量。
Abstract:
Objective To explore the application and effect of failure mode and effect analysis(FMEA)in the prevention of stress injury in weight loss surgery.Methods Patients undergoing weight loss surgery from January 2018 and December 2019 in a hospital and their surgery nurses were included as the control and observation groups by the convenience sampling method.The routine nursing procedure of stress injury was adopted in the control group(n=34).The observation group(n=41)adopted the stress injury nursing process optimized by FMEA to carry out targeted rectification of the selected high-risk failure modes.The changes of RPN value of high-risk failure mode,the incidence of high-risk failure mode and the incidence of stress injury in patients in the control group and the observation group were statistically analyzed.Results After FMEA optimization,the RPN value of high-risk failure mode,the incidence of high-risk failure mode and the incidence of the stress injury in patients in the observation group were significantly lower than those in the observation group(all P<0.05).Conclusions FMEA can effectively screen the high-risk failure modes that affect the occurrence of stress injury in patients undergoing weight loss surgery,optimize the nursing process to improve the quality of nursing.
