摘要:
目的 探讨夫妻疾病沟通问题和疾病接受度在乳腺癌术后患者自我感受负担和疏离感中的链式中介作用。方法 便利选取2021年6月至2022年2月在郑州市3所三级甲等医院乳腺外科、普外科治疗的248例乳腺癌术后患者为调查对象，使用自我感受负担量表、夫妻癌症相关沟通问题量表、疾病接受度量表、一般疏离感量表对其进行调查。结果 乳腺癌术后患者自我感受负担、夫妻疾病沟通问题、疾病接受度与疏离感之间存在相关性(均P<0.01)。自我感受负担不仅可以直接影响疏离感(95%CI:0.012～0.223)，且还可以通过夫妻疾病沟通问题和疾病接受度的链式中介作用影响疏离感(95%CI:0.024～0.156)。结论 护理人员可通过改善患者的夫妻疾病沟通问题和提高其疾病接受度，降低其自我感受负担对疏离感的不良影响。
Abstract:
Objective To explore the chain mediating effect of illness communication problems within couples and illness acceptance between self-perceived burden and alienation among postoperative breast cancer patients.Methods Convenient sampling was used to select 248 postoperative breast cancer patients from the departments of breast and general surgery in three tertiary hospitals in Zhengzhou City from June, 2021 to February, 2022.The Self-Perceived Burden Scale, Cancer-Related Communication Problems within Couples Scale, Chinese version of Acceptance of Illness Scale and Generalized Social of Alienation Scale were adopted for investigation.Results There were correlations between self-perceived burden, illness communication problems within couples, illness acceptance and alienation(all P<0.01).Self-perceived burden was found to directly affect alienation(95%CI: 0.012～0.223), and also indirectly affect alienation through the chain mediating role of illness communication problems within couples and illness acceptance(95%CI:0.024～0.156). Conclusions Nursing staff can reduce the negative impact of self-perceived burden on alienation by solving illness communication problems within couples and improving illness acceptance.
