摘要:
目的 探讨先天性唇腭裂患儿主要照顾者负担状况及其影响因素。方法 便利抽样法选取2019年5月至2021年3月收治于上海市某三级甲等医院手术治疗的207例唇腭裂患儿主要照顾者为研究对象,采用Zarit照顾者负担量表(Zarit caregiver burden interview,ZBI)对其进行调查分析。结果 唇腭裂患儿主要照顾者ZBI总分为(27.33±10.437)分,其中:很少负担39例(18.84%)、轻度负担141例(68.12%)、中度负担25例(12.08%),重度负担2例(0.96%); 回归分析显示,照顾者文化程度、家庭参与照顾人数、是否西部地区、医疗支付手段、家庭经济收入、报销政策了解程度、与外界交流患儿病情、愧疚和自责情绪、家中长辈看待唇腭裂患儿态度等9个因素影响主要照顾者负担(均P<0.05)。结论 影响先天性唇腭裂患儿主要照顾者负担的因素是多方面的,应从不同地域、文化角度、家庭情况等综合因素制定改善措施,提高他们对唇腭裂畸形的客观认识、增强应对能力。
Abstract:
Objective To explore the status quo and influencing factors of the burden among primary caregivers of children with congenital cleft lip and palate.Methods Convenience sampling method was adopted to select 207 primary caregivers of children with cleft lip and palate who underwent surgery in a tertiary hospital in Shanghai City from May,2019 to March,2021.The Zarit Caregiver Burden Interview(ZBI)was used for investigation and analysis.Results The ZBI total score among primary caregivers of children with congenital cleft lip and palate was(27.33±10.437); of them,39(18.84%)caregivers reported little burden,141(68.12%)reported mild level of burden,25(12.08%)reported moderate level of burden,and 2(0.96%)reported severe level of burden.Regression analysis showed that caregivers' education level,number of family caregivers,whether from western regions,means of medical payment,family income,understanding of reimbursement policy,communications with others about children's disease,emotions of guilt and self-blame,and attitude of elder family members towards the children with cleft lip and palate were the nine factors that affected primary caregivers' burden(all P<0.05).Conclusions There are various factors affecting the burden of primary caregivers of children with congenital cleft lip and palate.Improvement measures should be formulated based on comprehensive factors,such as different regions,cultural perspectives,and family circumstances,so as to improve their objective understanding of cleft lip and palate deformities and enhance their coping ability.
