摘要:
目的 探讨恐惧疾病进展和抑郁在症状负担和生活质量之间的链式中介作用。方法 便利抽样选取2020年11月至2021年4月期间在四川省3所三级甲等医院肾病内科治疗的309例血液透析患者为研究对象,采用血液透析患者症状量表、恐惧疾病进展简化量表、患者健康问卷抑郁量表、健康调查简表(the MOS item short from health survey,SF-36)的简化版本对其进行调查。结果 本组患者症状负担与恐惧疾病进展、抑郁评分呈正相关(P<0.01),与生活质量评分呈负相关(P<0.01); 恐惧疾病进展和抑郁评分与生活质量评分与呈负相关(P<0.01)。路径分析显示,维持性血液透析患者症状负担直接影响生活质量; 同时还存在2条路径:症状负担通过抑郁影响生活质量,中介效应为-0.036 [95%CI(-0.068,-0.012)]; 症状负担通过恐惧疾病进展和抑郁的链式中介作用影响生活质量,链式中介效应为-0.007[95%CI(-0.014,-0.001)]。结论 症状负担不但直接对患者生活质量产生影响,还通过恐惧疾病进展和抑郁间接影响患者的生活质量; 医护人员可在缓解患者恐惧疾病进展和抑郁情绪的基础上,降低症状负担对患者生活质量的不良影响。
Abstract:
Objective To explore the chain mediating effect of fear of disease progression and depression between symptom burden and quality of life.Methods Convenient sampling was used to select 309 hemodialysis patients from the Department of Nephrology of three tertiary hospitals in Sichuan Province from November,2020 to April,2021.The Dialysis Symptom Index,the Fear of Progression Questionnaire-Short Form,the Patient Health Questionnaire-9,and the SF-12 Quality of Life Scale were used for investigation.Results The score of symptom burden was positively correlated with the scores of fear of disease progression and depression(P<0.01),and was negatively associated with the quality of life score(P<0.01).The scores of fear of disease progression and depression were negatively correlated with the quality of life score(P<0.01).Path analysis showed that the symptom burden of maintenance hemodialysis patients directly affected the quality of life.There were two indirect paths:symptom burden affected quality of life through depression,with the mediating effect to be -0.036 [95%CI(-0.068,-0.012)]; symptom burden affected quality of life through the chain mediating effect of fear of disease progression and depression,and the chain mediating effect was -0.007 [95%CI(-0.014,-0.001)].Conclusions Symptom burden not only directly affects patients' quality of life,but also indirectly affects patients' quality of life through fear of disease progression and depression.Medical staff can reduce the adverse effects of symptom burden on patients' quality of life by relieving patients' fear of disease progression and depression.
