摘要:
目的 探讨静脉血栓栓塞症(venous thromboembolism,VTE)预防循证护理方案在骨科患者中的应用效果。方法 采用便利抽样法选取上海市某三级甲等医院骨科患者306例,按照入院时间先后分为试验组和对照组,试验组采用VTE预防循证护理方案进行护理,对照组采用常规护理,通过血栓风险评估及时率和准确率、患者对预防措施相关健康教育的掌握率、术后物理预防措施依从性等指标进行评价。结果 试验组患者入院血栓风险评估及时率、入院和术后评估准确率以及对物理预防相关健康教育掌握率和术后物理预防依从性差异均优于对照组,差异均有统计学意义(均P<0.001); 两组患者VTE发生率差异无统计学意义(P>0.05)。结论 基于最佳证据构建的VTE护理预防方案为临床护士预防VTE提供的护理实践参考依据,有助于规范临床护理行为,提高护理质量,具有可行性。
Abstract:
Objective To explore the application effects of the evidence-based nursing program for prevention of venous thromboembolism(VTE)in orthopedic inpatients.Methods Convenient sampling was used to select 306 orthopedic inpatients from a tertiary hospital in Shanghai City.They were divided into the experimental group and the control group according to the admission time.The experimental group received the evidence-based nursing program for VTE prevention,and the control group received routine nursing care.The application effects were evaluated by the timeliness and accuracy rate of VTE risk assessment,patients' knowledge level of health education on preventive measures,and their compliance with postoperative physical precautions.Results The timeliness rate of VTE risk assessment at admission,the accuracy rate of assessment at admission and after operation,the knowledge level of health education on physical prevention,and the compliance with postoperative physical precautions of the experimental group were significantly higher than those of the control group(all P<0.001).There was no statistical difference in the incidence of VTE between the two groups(P>0.05).Conclusions The nursing program based on best evidence for VTE prevention provides practical references for clinical nurses to prevent VTE.It is helpful and feasible to standardize clinical nursing behaviors and improve the quality of nursing.
