摘要:
目的 研制适用于老年医疗护理机构老年人的身体约束评估表,为落实身体约束最小化原则提供参考。方法 2019年3月至2020年10月,便利抽样法选取上海市老年护理管理质控中心管辖的66所老年医疗护理机构入住的3662名老年人为研究对象,采用自行设计的老年人身体约束使用现况调查表对其进行调查,并通过分析老年人身体约束的高危因素建立评估模型。通过ROC曲线验证评估模型的预测效能,并进行界值选择的验证。结果 性别、年龄、意识、肌力、行为、治疗/设备、日常生活活动能力、认知功能、跌倒史、管道滑脱史是老年医疗护理机构老年人身体约束的10项高危因素。当界值为12时,躁动组灵敏度为97.33%、特异度为10.26%; 平静组灵敏度为50.79%、特异度为81.08%。结论 研制的老年医疗护理机构老年人身体约束评估表具有可行性、适用性,可为遵循身体约束最小化原则提供参考依据。
Abstract:
Objective To develop the assessment form for the elderly on the use of physical restraints in the geriatric medical care institutes,and to provide reference for the realization of physical restraint minimization action.Methods From March 2019 to October 2020,3662 elderly patients in 66 geriatric medical care institutions were selected by the convenience sampling method.A self-designed questionnaire was used to investigate the use of physical restraints in the elderly,and the high risk factors of physical restraints in the elderly were analyzed to establish an evaluation model.The prediction efficiency of the evaluation model was verified by ROC curve,and the verification of boundary value selection was carried out.Results Gender,age,consciousness,muscle strength,behavior,treatment/device,Activity of Daily Living Scale(ADL)rating,cognitive function,history of fall and history of tube-off were the 10 high risk factors of physical restraints in the geriatric care institutions. When the threshold score was 12,the sensitivity and specificity were 97.33% and 10.26% in the agitation group,while 50.79% and 81.08% in the calm group.Conclusions The developed physical restraint assessment form for the elderly in the geriatric medical care institutions is feasible and applicable,which can provide reference basis for following the principle of physical restraint minimization action.
