摘要:
目的 探析癌症患者治疗决策过程并构建其理论框架。方法 基于程序性扎根理论研究方法对32例癌症患者进行半结构式访谈,通过编码和持续性比较方法对资料进行分析。结果 析出主题“关系性自主治疗决策行为”的12个类属,包括:“弃权、授权、赋权”3个参与策略,“个人因素、人际间因素、临床情境因素、社会文化因素”4个情境条件,“信息支持、沟通支持、情感支持”3个干预条件,“患者与他人围绕决策问题互动”1个因果条件,“决策控制感”1个参与结果。结论 癌症患者关系性自主治疗决策行为理论框架对促进患者参与决策相关研究及实践具有指导意义。
Abstract:
Objective To explore the process of treatment decision-making among cancer patients and construct its theoretical framework.Methods The procedural grounded theory method was adopted and 32 cancer patients were selected for semi-structured interviews.The coding and constant comparative analysis methods were used for data analysis.Results Twelve categories on relational autonomy treatment decision-making behaviors were extracted,including three participating strategies(abstention,delegation and empowerment),four contextual conditions(individual,interpersonal,clinical situational and sociocultural factors),three intervention conditions(information,communication and emotional support),one causal condition(patients' interaction with others on decision-making)and one consequence(control of decision-making).Conclusions This theoretical framework of relational autonomy treatment decision-making behaviors among cancer patients can guide the research and practice of promoting patients' participation in decision-making.
