摘要:
目的 验证跌倒警觉度量表(the self-awareness of falls in elderly scale,SAFE)在养老机构老年人群中的信效度,并探讨量表的最佳界值。方法 便利抽样法选取养老机构300位老年人采用跌倒警觉度量表进行调查,评价问卷的信效度,并分析受试者工作特性曲线(receiver operating characteristic curve,ROC)确定量表的最佳界值。结果 SAFE量表的Cronbach's α系数为0.943。条目得分与总分的相关系数为0.462～0.847(P<0.01)。重测信度为0.900。探索性因子分析共得到4个公因子,累积方差贡献率为74.609%。验证性因子结果显示模型拟合良好[χ2/df=2.135; 近似误差均方根(root mean square error of approximation,RMSEA)=0.076,增值适配度指数(incremental fit index,IFI)=0.941,非规范拟合指数(tucker-lewis coefficien,TLI)=0.931,相对拟合度指数(comparative fit index,CFI)=0.940]。量表4个因子的组合信度(critical ratio,CR)均大于0.7,平均方差萃取量(average variance extract,AVE)均大于0.5。SAFE最佳界值为54分,ROC曲线下面积为0.803,此时的敏感度为61.0%,特异度为88.4%。结论 SAFE具有较好的信效度,推荐最佳界值为54,可用于评价养老机构老年人群的跌倒警觉度。
Abstract:
Objective To test the reliability and validity of the Self-Awareness of Falls in Elderly(SAFE)scale among elderly in the nursing homes,and to determine the best cut-off value.Methods By convenience sampling method,300 elderly in nursing homes were selected and investigated to evaluate the reliability and validity of the SAFE scale,and the receiver operating characteristic curve(ROC)was analyzed to determine the best cut-off value of the scale.Results The Cronbach's α coefficient of the SAFE scale was 0.943.The correlation coefficients among the item score and the total scale ranged from 0.462 to 0.847(P<0.01).The test-retest reliability was 0.900.The exploratory factor analysis(EFA)obtained 4 common factors,and the cumulative variance interpretation rate was 74.609%.The confirmatory factor analysis(CFA)showed that the χ2/df value was 2.135; the IFI,TLI,CFI values were 0.941,0.931,0.940,respectively; and the RMSEA was 0.076.The critical ratios among the 4 common factors in SAFE scale were over 0.7; all of the average variance extraction values were over 0.5.The best cut off points for SAFE scale was 54,the areas under the ROC curve for SAFE scale was 0.803 with the sensitivity of 61.0% and specificity of 88.4%.Conclusions SAFE scale has a good reliability and validity,with the optimal cut-off value of 54; it can be used to evaluate the self-awareness of falls in elderly of nursing homes.
