摘要:
目的 探讨心理弹性在癫痫患者病耻感和生活质量间的中介作用。方法 采取便利抽样的方法,选取2019年1月至2020年10月于江苏省盐城市某三级甲等医院神经内科住院治疗的290例癫痫患者作为研究对象,采用一般资料调查表、Kilifi癫痫患者羞耻感量表(Kilifi stigma scale for epilepsy,KSSE)、心理弹性量表(Connor-Davidson resilience scale,CD-RISC)及癫痫患者生活质量量表31项(quality of life in epilepsy inventory-31 scale,QOLIE-31)对其进行调查。结果 癫痫患者病耻感得分为(11.32±2.84)分、心理弹性得分为(50.56±5.46)分、生活质量得分为(40.24±6.02)分; 病耻感与生活质量、心理弹性呈负相关(r=-0.361,P<0.01; r=-0.297,P<0.01),心理弹性与生活质量呈正相关(r=0.345,P<0.01); 心理弹性在病耻感与生活质量间起到中介作用,中介效应占总效应的25.4%。结论 癫痫患者病耻感和心理弹性可预测其生活质量,较高水平的病耻感可降低患者的生活质量,可通过提高患者心理弹性水平的间接作用改善其生活质量。临床护理工作者应对癫痫患者给予更多的心理照护,加强患者坚韧、自强和乐观性格的培养。
Abstract:
Objective To explore the relationship among psychological resilience,stigma and quality of life in patients with epilepsy.Methods By convenience sampling method, a total of 290 epileptic patients who were hospitalized in the Department of Neurology,a tertiary hospital in Yancheng,Jiangsu Province from January 2019 to October 2020 were selected as the research objects.The general information questionnaire,Kilifi Stigma Scale for Epilepsy,Connor-Davidson Resilience Scale and quality of life in epilepsy inventory-31 scale were used for investigation.Results The scores of stigma,resilience and quality of life were(11.32±2.84),(50.56 ± 5.46)and(40.24 ± 6.02),respectively; there were negative correlations among stigma,quality of life and resilience(r=-0.361,P<0.01; r=-0.297,P<0.01),and there was a positive correlation between resilience and quality of life(r=0.345,P<0.01). Resilience played a mediating role between stigma and quality of life, accounting for 25.4% of the total effect. Conclusions The stigma and resilience of epilepsy patients can predict their quality of life,higher level of stigma can reduce the quality of life of patients,and their quality of life can be improved through the indirect effect of improving the level of resilience.Clinical nurses should give more psychological care to patients with epilepsy,strengthen the exercise of patient's tenacity,self-improvement and optimistic character.
