摘要:
目的 探讨毒性反应及负性情绪对鼻咽癌患者调强放疗后睡眠质量的影响。方法 采用便利抽样法选取广州市某三甲医院鼻咽癌调强放疗后患者200例。采用一般资料问卷、匹兹堡睡眠质量指数量表及简式简明情绪状态问卷进行调查。结果 鼻咽癌患者调强放疗后睡眠质量平均分为(7.82±3.24)分,有64%(128例)出现睡眠障碍,其影响因素包括性别、年龄、教育水平、居住地、工作状态、婚姻状况、家庭人均月收入、放疗剂量、放疗后时间、慢病史、疲乏、张口困难、口干、记忆力下降、头晕头痛以及负性情绪(P<0.05)。记忆力下降、张口困难及疲乏对睡眠质量均有显著预测作用,并且负性情绪在毒性反应和睡眠质量之间起中介作用。结论 医护人员应采取有效干预措施减轻患者的化疗毒性反应,缓解其负性情绪,从而提高其睡眠质量。
Abstract:
Objective To investigate the effects of toxic reactions and negative emotions on sleep quality of patients with nasopharyngeal carcinoma after intensity-modulated radiotherapy.Methods By convenience sampling method,200 patients with nasopharyngeal carcinoma after intensity-modulated radiotherapy from a tertiary hospital in Guangzhou were selected and investigated by the General Information Questionnaire,Pittsburgh Sleep Quality Index,and Profile of Mood States-Short Form.Results The mean score of sleep quality was(7.82±3.24)with 64% of patients(128 cases)having sleep disturbance.The influencing factors of sleep quality included gender,age,education level,place of residence,working status,marital status,family per capita monthly income,radiotherapy dose,time after radiotherapy,slow medical history,fatigue,difficulty opening mouth,dry mouth,memory decline,dizziness and headache and negative emotion(all P<0.05).Memory loss,trismus,and fatigue had significant predictive effects on sleep quality; the negative emotion has mediating effect between these toxicities and sleep quality.Conclusions Medical professionals should take effective measures to alleviate the patients' toxicities,then to reduce their negative emotions and improve their sleep quality eventually.
