摘要:
目的 了解经外周静脉置入中心静脉导管(peripherally inserted cen-tral catheters,PICC)专科护士在为癌症患者置管-维护-拔管过程中运用叙事护理方法的工作体验、成长收获和开展叙事护理面临的困难和挑战,为叙事护理的推广及护理专科人才培养提供依据。方法 目的性抽样选择山西省某三级甲等医院为患者实施叙事护理的PICC专科护士11名进行半结构式访谈,通过Colaizzi的现象学7步分析法进行资料分析。结果 PICC专科护士开展叙事护理体验共提炼出3个主题:叙事护理中的体验和感受; 叙事护理中自身的成长与收获; 进一步开展叙事护理面临的挑战与困难。结论 在癌症患者中开展叙事护理可促进PICC专科护士的成长,提高工作成就感和职业价值感,提升患者满意度和医生认可度。管理者应明确专科护士工作内容和岗位职责,采取相关措施改善人员配置和交流平台,促进叙事护理在癌症患者中进一步的开展。
Abstract:
Objective To understand the work experience,personal growth and achievements of peripherally inserted central catheters(PICC)nursing specialists using the narrative nursing approach in the process of catheterization,maintenance,and extubation for cancer patients,as well as the difficulties and challenges faced by narrative nursing,so as to provide a basis for the promotion of narrative nursing and the development of clinical nurse specialists.Methods A semi-structured interview was conducted with 11 PICC clinical nurse specialists who performed narrative nursing for cancer patients in a hospital.Data were analyzed using Colaizzi 7-step phenomenological analysis method.Results Three themes emerged from the narrative nursing experiences of the PICC clinical nurse specialists:the experiences and feelings of the specialists; personal growth and achievements in the process; and the challenges and difficulties that remain in further developing narrative nursing.Conclusions Narrative nursing in cancer patients can not only promote the personal growth of PICC nurses and improve their comprehensive strength,but also enhance their sense of achievement and professional value.It also facilitates the patients' satisfaction and doctors' recognition.Managers should clarify the job responsibilities of specialized nurses,take relevant measures to improve staffing,build communication platforms,and promote the further development of narrative nursing in caring for cancer patients.
