摘要:
目的 探讨基于网格化管理的持续质量改进模式(grid management-PDCA,GM-PDCA)在静脉血栓栓塞症(venous thromboembolism,VTE)防控中的应用效果,以期为提高护理质量和管理效率提供现实依据。方法 2019年1-12月,便利抽样法选取某院的住院患者为对照组,即GM-PDCA实施前组。同法选择2020年1-12月的住院患者为观察组,即GM-PDCA实施后组。2020年1月构建VTE专业、质量控制和信息团队,实施护理部-片区-病区三级网格化管理,开展VTE评估、宣教、督导等质量改进活动。比较GM-PDCA前后两组住院患者的VTE风险评估率及正确率、预防措施落实率及医院相关性VTE发生率。结果 GM-PDCA实施后,VTE风险评估率及正确率、预防措施落实率均上升,医院相关性VTE发生率下降(均P<0.001)。结论 基于GM-PDCA模式的持续质量改进有助于提高全院VTE防控水平,降低医院相关性VTE发生率。
Abstract:
Objective To explore the effect of continuous quality improvement of prevention and control of venous thromboembolism(VTE)based on Grid Management Plan-Do-Check-Active(GM-PDCA)Model, and to provide reference for improvement of nursing quality and efficiency.Methods From January to December 2019, inpatients in a hospital were selected by convenience sampling method as the control group, that is, the group before the implementation of GM-PDCA.In the same way, hospitalized patients from January to December 2020 were selected as the observation group, namely the post-implementation GM-PDCA group.Build VTE professional, quality control and information team, implement the three-level grid management of nursing department, area and ward, and carry out quality improvement activities such as VTE risk assessment, education and supervision.The risk assessment rate and accuracy rate of VTE, the implementation rate of preventive measures and the incidence rate of hospital-related VTE were compared between the two groups before and after GM-PDCA.Results After quality improvement, the risk assessment rate, the correct rate of risk assessment, the implementation rate of preventive measures of VTE increased, the incidence of hospital related VTE decreased(all P<0.001).Conclusions Continuous quality improvement based on GM-PDCA model can improve the prevention and control level of VTE and reduce the incidence of hospital-related VTE.
