摘要:
目的 了解我国伤口治疗师主导的医护团队合作开展现状及其影响因素,为促进医护间有效合作提供参考。方法 采用目的抽样法选取来自上海的7所三级甲等医院,已开展的伤口治疗师主导的医护团队中的16名成员(临床医生、伤口治疗师和管理者)为研究对象,进行半结构式访谈,采用Colaizzi 7步分析法对资料进行分析。结果 访谈结果共提炼出3个主题9个亚主题:伤口治疗师主导的团队合作现状(合作现状整体良好、具有较好的职业发展前景、资源不足有待改善); 开展的促进因素(领导者的角色胜任能力、及时有效的信息传递); 开展的阻碍因素(合作流程和职责不清晰、伤口病案管理信息系统不完善、监督反馈不足、存在专业意见分歧)。结论 医护人员认为伤口治疗师主导的团队合作现状整体良好,合作态度较为积极,但在开展中面对一定困境,存在提升空间。各级管理者需关注并完善治疗师主导的团队合作模式,促进有效伤口管理质量和患者安全。
Abstract:
Objective To investigate the status quo of physician-and-nurse cooperation led by wound therapists in China and its influencing factors and thus to provide a reference for the effective cooperation.Methods A semi-structure interview was conducted in 16 medical staffs including physicians,wound therapists and administrators who were selected from 7 tertiary hospitals in Shanghai by purposive sampling method.The data was analyzed by Colaizzi method.Results 3 themes and 9 sub-themes were extracted from the interview data analysis namely current situation(a good current situation,a good career development prospect,insufficient resources),facilitating factors(leader’s competency,timely and effective information transmission),hindering factors(unclear cooperation process and responsibilities,imperfect wound management information system,lack of supervision and feedback,inter-professional disagreement).Conclusions The overall current situation is quite good.Medical staffs have a positive attitude towards teamwork led by wound therapists.However,difficulties still exist and there is still room for improvement.Administratiors at all levels need to pay more attention to the practice and to improve wound management quality and ensure patient safety.
