摘要:
目的 评价以家庭为中心的个案管理在1型糖尿病患儿中的应用效果。方法 便利抽样选取重庆市某三级甲等儿童医院2018年9月至2019年7月内分泌科收治的62例1型糖尿病患儿为研究对象。试验组(33例)实施以家庭为中心的个案管理照护,对照组(29例)实施儿童糖尿病临床路径照护,干预时间均为6个月。比较两组患儿糖化血红蛋白、凌晨血糖、空腹血糖、睡前血糖和生存质量。结果 干预6个月后,试验组患儿凌晨、空腹和睡前血糖均低于对照组(均P<0.05),两组糖化血红蛋白差异无统计学意义(P>0.05); 试验组生存质量自评及代评量表得分高于对照组,除生理功能外(P>0.05),其他维度差异均有统计学意义(均P<0.05)。结论 以家庭为中心的个案管理有助于控制患儿糖代谢指标,改善患儿生存质量,值得推广应用。
Abstract:
Objective To evaluate the effect of family-centered case management for children with type 1 diabetes.Methods By convenience sampling method,62 children with type 1 diabetes admitted to the Department of Endocrinology in a tertiary children’s hospital from September 2018 to July 2019 were selected as the research object.The experimental group(33 cases)received family-centered case management,and the control group(29 cases)received routine care according to the clinical path of childhood diabetes.The intervention duration was 6 months.HbA1c,early morning blood glucose,fasting blood glucose,bedtime blood glucose and quality of life were compared between the two groups.Results After intervention for 6 months,the early morning blood glucose,fasting blood glucose and bedtime blood glucose of the children in the experiemental group were lower than those in the control group(all P<0.05),and there was no statistically significant difference in HbA1c between the two groups(P>0.05).Self-assessment and parental-representative quality of life scale in the experimental group performed significantly higher score than those in the control group(all P<0.05),except the physiological function(P>0.05).Conclusions Family-centered case management can enhance control of glucose metabolism and improve the quality of life of children,which is worthy of popularization and application.
