摘要:
目的 描述社区老年人主观认知下降(subjective cognitive decline,SCD)发生现状,从生理、心理、生活方式等方面分析其影响因素。方法 2019年3-12月,便利抽样选取北京市2所社区卫生服务中心就诊的337名老年人为研究对象,采用一般情况调查问卷、主观认知下降问卷、蒙特利尔认知评估量表、日常生活能力量表、老年抑郁量表、焦虑自评量表对其进行调查。 结果 社区老年人SCD有111例(32.9%)。回归分析显示,睡眠障碍、焦虑、抑郁、负性生活事件是社区老年人SCD的危险因素; 日常生活能力是社区老年人SCD的保护因素。结论 社区老年人SCD发生率较高,医护人员应加强对社区老年人的认知评估,尤其重视其心理和生活方式方面,尽早采取有针对性的护理干预措施,延缓其向痴呆症发展。
Abstract:
Objective To describe the status quo of subjective cognitive decline(SCD)among community-dwelling elder adults and explore the risk factors from the perspective of physiology,psychology and lifestyle.Methods A total of 337 older adults were recruited from two community health clinics by convenience sampling method from March to December 2019.All participants were investigated by the General Information Questionnaire,Montreal Cognitive Assessment,Activities of Daily Living Scale,Geriatric Depression Scale,and Self-rated Anxiety Scale.Results The prevalence of SCD was 32.9%(n=111)in the study sample.Logistic regression analysis showed that sleep disorder,stressful life events,anxiety,and depression were risk factors of SCD,and activities of daily living were protective factors of SCD.Conclusions The incidence of SCD among community-dwelling older adults was high.Medical staff should strengthen the assessment of SCD,pay more attention to lifestyle and psychological factors and adopt targeted nursing interventions to prevent its progression to dementia.
