摘要:
目的 探讨肿瘤化疗患儿发热性中性粒细胞减少(febrile neutropenia,FN)的影响因素,构建风险预测模型。方法 前瞻性收集2020年12月至2021年12月某儿童医院肿瘤化疗患儿的相关临床资料,依据儿童报告版通用不良反应术语标准评估FN的相关症状,分析其FN发生的独立危险因素,并构建风险预测模型。采用Hosmer-Lemeshow检验和校正曲线评估预测模型的区分度和校准度。结果 最终进入预测模型的因子为性别、癌症类型、中央静脉导管类型、化疗预计持续时间、疼痛评分均值以及手脚无力评分均值,构建的列线图模型经内部验证,得到的一致性系数(C-index)为0.773[95%CI(0.710,0.836)],Hosmer-Lemeshow检验P=0.759。K-S检验显示列线图预测的FN发生概率与实际发生的FN概率间一致性较好。结论 预测模型将协助医护人员早期、无创、经济、便捷的对肿瘤化疗患儿进行FN发生风险的预判。
Abstract:
Objective To investigate the influencing factors of febrile neutropenia(FN)in children with tumor and to establish a risk prediction model.Methods The relevant clinical data were prospectively collected for children with tumors chemotherapy at a children's medical center from December 2020 to December 2021 and were assessed the items of FN related symptoms in Pediatric patient-reported Outcomes Common Terminology Criteria Adverse Events(Ped-PRO-CTCAE),and to analyzed the independent risk factors for FN occurrence,and constructed a risk prediction model.Bootstrap(repeated 1000 times)was used for internal validation.C-index and Hosmer-Lemeshow test and calibration curve were used to evaluate the differentiation and calibration of prediction models.Results The factors that were finally included in the prediction model were gender,type of cancer,type of central venous catheter,expected duration of chemotherapy,mean value of pain score and mean value of weakness score of hands and feet.The consistency coefficient(C-index)of the constructed rosette model was 0.773[95%CI(0.710,0.836)] after internal verification.,and Hosmer-Lemeshow test P=0.759.The K-S test show that the probability of FN occurrence predicted by the line graph is consistent with the actual probability.Conclusions This prediction model can assist medical staff in early,noninvasive,economical and convenient prediction of FN risk in children undergoing tumors chemotherapy.
