摘要:
目的 探讨矫正胎龄32周早产儿非营养性吸吮现状及影响因素,以期为早产儿非营养性吸吮干预方案的制订提供参考。方法 采用便利抽样法选取入住某院新生儿病区的矫正胎龄32周早产儿163例为研究对象,采用自制的一般情况调查表以及生理参数诱发诊断仪对其调查。结果 163例早产儿监测2 min的平均吸吮压力为(6.23±1.19)kPa,吸吮持续(35.01±8.24)s,吸吮频率为(18.60±5.68)次/min。不同体质量(矫正胎龄32周时)、是否接受口腔运动干预的早产儿,其平均吸吮压力、频率差异均有统计学意义(均P<0.05); 不同体质量(矫正胎龄32周时)、非营养性吸吮时体位的早产儿,其吸吮持续时间及频率差异均有统计学意义(均P<0.05)。不同吸氧状态的早产儿,其吸吮持续时间差异有统计学意义(P<0.05)。多元回归分析显示,不同体质量(矫正胎龄32周时)、是否接受口腔运动干预是平均吸吮压力、频率的影响因素(均P<0.05); 不同体质量(矫正胎龄32周时)、非营养性吸吮时体位等是吸吮持续时间、频率的影响因素(均P<0.05); 吸氧状态是吸吮持续时间的影响因素(P<0.05)。结论 医护人员应积极促进早产儿体质量增长,积极开展口腔运动干预,在喂养过程中采用侧卧位,并缩短其用氧时长,从而提高其吸吮能力。
Abstract:
Objective To explore the status quo of non-nutritive sucking of preterm infants at a post-conceptual age of 32 weeks and its influencing factors.Methods By the convenience sample method,163 hospitalized preterm infants at a post-conceptual age of 32 weeks were selected as research subjects,and a survey was conducted with personal information questionnaires and physiological parameters induced diagnostic apparatus.Results The average sucking pressure within 2 min of 163 research subjects was(6.23±1.19)kPa,each sucking duration within 2 min was(35.01±8.24)s,and sucking frequency within 2 min was(18.60±5.68)times/min.There were significant differences in sucking duration and frequency in premature infants with different body weight at a post-conceptual age of 32 weeks and postural during non-nutritive sucking(all P<0.05).There was a statistically significant difference in the sucking duration of the research subjects(P<0.05).Multivariate Logistic Regression showed that weight of preterm infants at a post-conceptual age of 32 weeks and mouth movement intervention were influencing factors of the average sucking pressure(all P<0.05); weight of preterm infants at a post-conceptual age of 32 weeks,and postural during non-nutritive sucking were influencing factors of the average sucking duration(all P<0.05); and oxygen status was the influencing factor of the sucking duration(P<0.05).Conclusion Medical staff should actively promote the weight gain of premature infants,actively carry out mouth movement intervention,use lateral position during feeding,and shorten the duration of oxygen consumption,so as to improve their sucking ability.
