摘要:
目的 系统评价心血管疾病(cardiovascular disease,CVD)患者及家属参与预立医疗照护计划(advance care planning,ACP)体验的质性研究,为ACP在CVD患者及家属中的应用提供循证依据。方法 计算机检索MEDLINE、PubMed、Web of Science、Embase、Cochrane library、ProQuest、中国知网、万方和中国生物医学文献数据库中关于CVD患者及家属参与ACP体验的相关质性研究,检索时限从建库至2022年4月,采用统一标准对文献进行质量评价,运用汇集性整合方法进行结果整合。结果 共纳入7篇文献,提炼出22个明确的研究结果,归纳为4个类别,进一步综合得到2个整合结果:个性化ACP可缓解负性情绪,明确临终意愿; 对ACP具有刻板印象,缺乏信息、情感及经济支持。结论 CVD相关ACP需遵从患者及家属偏好,高质量ACP需完善的政策及多学科人才提供支持。
Abstract:
Objective To conduct a meta-synthesis of the experience of patients with cardiovascular diseases(CVD)and their relatives in advance care planning(ACP)and provide evidence-based guidance for the development and implementation of ACP in patients with CVD.Methods MEDLINE,PubMed,Web of Science,Embase,Cochrane Library,ProQuest,CNKI,WanFang,SinoMed were searched to for the literature of the experience of cardiovascular diseases' patients and their relatives about ACP intervention from inception until April 2022.The quality of included studies was evaluated by unified criterion.Meta-aggregation was used to conduct the synthesis.Results A total of 7 literatures were included,and 22 clear research results were extracted.The results of similar combinations were summarized into 4 categories,and 2 integrated results were obtained,namely:personalized ACP which could alleviate negative emotions and clarify end-of-life wishes; stereotyped attitudes towards ACP and lacks information,emotional and financial support.Conclusions ACP implementation needs to respect patient and family preferences,and high-quality ACP requires supports from policies and multi-disciplinary talents.
