摘要:
目的 自行设计免缝式“甜甜圈+三明治”组合固定法,并评估其在超早产儿脐静脉置管(umbilical vascular catheterization,UVC)中的可应用性。方法 2019年1-12月,采用便利抽样法选取某医学中心超早产儿监护病房实施UVC置管的80例超早产儿为研究对象,按置管先后将其分为对照组和观察组各40例,对照组采用传统的荷包缝合式搭桥法对脐静脉导管进行固定,观察组采用免缝式“甜甜圈+三明治”组合法进行固定,比较两组患儿的脐静脉导管留置时间、导管移位、脐部渗血、脐炎及医用粘胶相关性皮肤损伤(medical adhesive-related skin injury,MARSI)的发生情况。结果 观察组患儿的脐静脉导管留置时间长于对照组,脐静脉导管移位、脐部渗血、脐炎及MARSI发生率均低于对照组,差异均有统计学意义(P<0.05或P<0.01)。结论 免缝式“甜甜圈+三明治”组合固定法在超早产儿脐静脉置管中应用效果良好,能有效延长脐静脉导管留置时间,减少脐静脉导管移位、脐部渗血、脐炎及MARSI的发生。
Abstract:
Objective To design the sutureless “doughnut & sandwich” combined fixation method and evaluate its applicability in umbilical vascular catheterization(UVC)for extremely premature infants.Methods Convenient sampling was used to select 80 extremely premature infants who underwent UVC in the Extremely Premature Neonatal Intensive Care Unit of a medical center from January to December,2019.These infants were divided into the control group and the observation group according to the order of catheterization,with 40 infants in each group.The control group received the traditional purse-string suture bypass method to fix the umbilical vein catheter.The observation group received the sutureless “doughnut & sandwich” combined method for fixation.The indwelling time of umbilical vein catheter,the incidence of catheter displacement,umbilical oozing,omphalitis and medical adhesive-related skin injury(MARSI)were compared between the infants of the two groups.Results The indwelling time of umbilical vein catheter in the observation group was significantly longer than that in the control group,and the incidence of umbilical vein catheter displacement,umbilical oozing,omphalitis and MARSI in the observation group were significantly lower than those in the control group(P<0.05 or P<0.01).Conclusions The sutureless “doughnut & sandwich”combined fixation method has good application effects in UVC for extremely premature infants.It can effectively prolong the indwelling time of umbilical vein catheter and reduce the incidence of umbilical vein catheter displacement,umbilical oozing,omphalitis and MARSI.
