摘要:
目的 通过构建最大覆盖选址模型,分析基于院外心搏骤停(out-of-hospital cardiac arrest,OHCA)的第一反应人空间需求配置,以有效缩短OHCA应急反应时间,提高院前急救和现场救护水平。方法 收集上海市浦东新区2012年1月至2016年12月OHCA院前急救病历信息,将OHCA位置信息录入Arc地理信息系统(geographic information system,GIS)中,并构建最大覆盖选址模型,输出不同OHCA覆盖率下第一反应人的需求量及对应位置。结果 当覆盖半径为100 m时,第一反应人可在指南推荐的救治时间内快步行走至OHCA患者身边,其需求量至少为4626人时才能覆盖95%的OHCA。若第一反应人借助自行车骑行至患者身边,其最大覆盖半径为300 m,此时需至少配置1626名第一反应人才能覆盖95%的OHCA。结论 第一反应人空间需求配置方案为建立第一反应人体系和现场救护新模式提供了参考依据,并提示在未来可以培训网格化管理人员作为第一反应人,为其合理配置交通工具,从而提高OHCA患者存活率和社区急救护理水平。
Abstract:
Objective To analyze the spatial requirement allocation of first responders for out-of-hospital cardiac arrest(OHCA)by building the maximal covering location problem(MCLP)model,so as to shorten the OHCA emergency response time effectively and improve pre-hospital emergency care and field first-aid level.Methods The pre-hospital emergency medical records of OHCA patients in Pudong New District Shanghai city from January,2012 to December,2016 were collected.The OHCA location information was inputted into the Arc geographic information system(GIS),and then used to build the MCLP model.The quantity demand and corresponding location of the first responders were outputted under the conditions of different OHCA coverage rates.Results When the coverage radius was 100m,the first responder could walk quickly to the OHCA patient within the rescue time recommended by the guidelines.The quantity demand of first responders should reach 4626 to realize 95% coverage rate of the OHCA patients.If the first responder rode a bike to the OHCA patient,and the coverage radius was 300 m,a least of 1626 first responders were required for allocation to realize 95% coverage rate of the OHCA patients.Conclusions The spatial requirement allocation program of first responders provided a reference for the establishment of the first responder system and the new mode of field emergency care.It suggests that gridding managers can be trained as the first responders in the future,and the vehicles should be reasonably allocated,so as to increase the survival rate of OHCA patients and improve the level of community emergency care.
